The Cub World Akela Camp Parents’ Guide and all the necessary forms are available here. The guide will explain what needs to be done prior to camp, health form requirements, when to arrive, what to bring (and not bring), and other important information.
All Cub World Akela Camp participants (youth and adult) must have a completed BSA Annual Health and Medical Record with Parts A, B and C completed. Part B of the form is the physical examination which must be signed by a certified and licensed health-care provider recognized by the BSA. Recognized providers are physicians (MD, DO), nurse practitioners, and physician’s assistants. Substitute forms will not be accepted.
Health forms must be mailed (do not send via email or fax) to the Daniel Webster Council Service Center no later than two weeks prior to your son’s session. Please indicate “Cub World Akela Camp” and the session number clearly when mailing in the health form.
